Hearing Requests

OAH can only accept 4 types of hearing requests online:  Most Health Care Authority (HCA) cases with the exception of Apple Health (MAGI appeals); Public Assistance cases; WA Cares-Beneficiary; and WA Cares-Provider.

Please use the Health Care Authority Request for Hearing form to request hearings for Health Care Authority decisions such as:  

  • Medical Assistance Eligibility and Alien Emergency Medical
  • Developmental Disabilities Community Services (DDCS/DDA) Financial Eligibility, DDA Individual and Family Services, DDA Medicaid Personal Care, DDA Waivers
  • Home and Community Living Administration (HCLA) Financial or Functional Eligibility
  • Managed Care Organization
  • Medicaid scope of coverage, transportation, or equipment
  • Transfers or discharge from Medicaid funded long-term care facilities

Please use the Public Assistance Request for Hearing form to request hearings regarding:

  • Decisions by the Community Services Office such as Food assistance and Temporary Assistance for Needy Families (TANF)
  • Community Options Program Entry System (COPES)
  • Denials of applications to become a Developmental Disabilities Administration (DDD/DDA) client
  • Vocational Rehabilitation (DVR)

Please use the WA Cares – Beneficiary form to request a hearing regarding DSHS decisions regarding applications for long-term care services and supports through the WA Cares Fund.

Please use the WA Cares – Provider form to request a regarding DSHS WA Cares services or provider applicants.

In most cases, OAH cannot directly accept requests for hearing.

If you disagree with an agency notice, read it carefully. It has instructions on how to appeal. It is your responsibility to follow those instructions. If you do not, you may lose your right to appeal.
