Hearing or Prehearing Check-in
This form may be used if your Notice of Hearing asks you to call OAH and provide a phone number.

This form must be submitted at least 10 minutes before your hearing. If your hearing has already started, you must immediately call (360) 407-2700 or (800) 583-8271.

Do not use this form if your Notice directed you to dial into a Webex conference call. For example, do not use this form if your appeal relates to Unemployment Insurance or other Employment Security Department matters.

When the Judge calls, it may show as an "unknown caller."
